Enter your hospital’s six-digit CMS Certification Number (CCN) into the form to verify the current information we have on file.

 If you are having issues enabling the macros please try the following: 

1. Save a copy of the excel file 
2. Open Windows File Explorer and go to the folder where you saved the file.
3. Right-click the file and choose Properties from the context menu.
4. At the bottom of the General tab, select the Unblock checkbox and select OK.
